State seeks input on rural health with online survey; Deadline is Oct. 3

INDIANAPOLIS — The Indiana Department of Health is distributing a survey to rural communities throughout Indiana to collect information about ways to improve rural health.
The responses will be used to prepare Indiana’s application for the Rural Health Transformation Program, created under the Working Families Tax Cuts Act to strengthen healthcare across rural America.
The survey only takes a few minutes to complete, and participants are welcome to skip any question that doesn’t seem to apply.
Responses are due Oct. 3.
